Indian Registration (for Bill C-3 Applicants only)

2011 Indian Act Amendments - Gender Equity in Indian Registration Act Application for Registration and Secure Certificate of Indian Status and the instructions for filling in the application.

​

Parental Consent Form - to be completed and returned along with the original birth certificate which must state the parents names.  Once completed documents must be mailed to the Kispiox Band to the attention of Sandi Reviakin.
